You can apply to take your exam online or via US mail. The online applications allow for immediate credit card payment processing. Paper applications should be used when paying with certified check or money order.

Testing Accommodations

Prometric provides a variety of accommodations to candidates that qualify under the American’s with Disabilities Act (ADA). Submit this form with your application to receive accommodations on exam day.

If you are a current CNA ready to renew, submit he below form at least 2 weeks prior to your certificate expiration.

If you have any questions regarding your certification status or renewal form processed, please contact our customer service team for the CT Registry at866.499.7485 from 8am – 6pm ET, M-F. Please allow 10 business days from mailing date for renewal processing.

 *Please note that beginning January 1, 2016, there will be a fee increase of $8 for the Clinical Skills Test.
