Reciprocity Procedure

How to become certified in Louisiana through reciprocity from another state:

1. Fill out the application found under the Candidate Resources section of our website (www.prometric.com/exams/nurseaide-la), selecting the Route 8 Reciprocity option.
2. Submit the application form, $35 application fee, and a copy of your SSN card and government issued Louisiana ID or military ID to Prometric. If your address is within 30 miles of LA, you will not be required to obtain a Louisiana ID to process your reciprocity.
3. Once the application has been verified, Prometric will send your information to LDH for final review and approval to be placed on the Louisiana Registry.

How to become certified through reciprocity in another state when moving from Louisiana:

1. The Louisiana Nurse Aide Registry will no longer accept or process requests for written verification of Certified Nurse Aide (CNA) status.
2. To obtain printable verification of registry listings for the Louisiana Nurse Aide Registry, please visit the LDH website at https://tlc.dhh.la.gov/. The registry can be searched using name, social security number, certification number, or any combination of.
   • The information available includes original certification date, current certification status, date of expiration, and certification number.
3. If information regarding CNA’s original certificate, transcript, certification examination scores, etc. is required, please reach out to your training facility. The LA Nurse Aide Registry does not keep copies or have access to these records.
